We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees.

ENROLLMENT
You are eligible to participate if you are full-time and work a minimum of 30 hours per week. Your coverage will be effective 1st of the month following 60 days from your date of hire. When you enroll in the benefits program, you may also cover your eligible dependents. Dependents include your legal spouse and children. Children eligibility is as follows -
- Medical | Dental The last day of the calendar year the child turns age 26
- Voluntary Life: Up to age 26
ENROLLMENT INSTRUCTIONS:

- Enter your User ID: First Initial + Last Name
- e. g.John Smith = JSmith
- Enter your Password: Last 4 of your SSN
- Follow instructions and enroll in your benefits
- Make sure to save your elections and print your confirmation statement.
The Benefit choices you make now will remain in effect for the entire year. You can, however, modify your elections under certain circumstances, called "Qualifying Events" These are events such as marriage, divorce, birth or adoption of a child, loss of eligibility under another plan. If you experience a qualifying event, you may make changes to your benefits within 30 days of the event or 60 days if the event is due to birth or adoption of a child.

MEDICAL/RX

Priority Health will continue to be the administrator for you medical and prescription coverage. The plans that will be offered effective January 1, 2023 are as follows:
- PPO 1500
- HMO HRA 4000
- SEMP HMO HRA 4000

Preventative care services are covered 100% in-network. Preventative services included women's preventative healthcare services, well-child visits, flu shots and routine physical exams.
HMO HRA Southeast MI Partners (SEMP) $4,000/$8,000
- You are responsible for ($1,500/$3,000)
- You do need to choose a PCP but you can self-refer in the HMO network.
- If you choose a Tier 1 provider, you will benefit from lower coinsurance, copays and deductibles. If you choose Tier 2, you will have higher cost share for care.
HMO HRA $4,000/$8,000
- You are responsible for ($1,500/$3,000)
- You do need to choose a PCP but you can self-refer in the HMO network.
PPO 1500 – In/Out of Network Coverage.

American Jetway Corporation provides ALL employees access to virtual visits with Teladoc. Teladoc provides members the opportunity to receive medical care from the comfort of your own home. With Teladoc, eligible employees and dependents have access to board certified doctors 24 hours a day, 7 days a week, 365 days per year.

American Jetway Corporation provides comprehensive coverage to help you and your family maintain good dental health. For 2023, the dental carrier will be Delta Dental. The PPO plan will be offered for 2023. Staying in-network will give you the most cost savings.

American Jetway Corporation provides you with financial protection options for life, accidental death & dismemberment, short term disability and long term disability with Mutual of Omaha
Life/AD&D
A sudden accident or death can leave you or your loved ones in a vulnerable position. Employees have the opportunity to enroll in Term Life and Accidental Death & Dismemberment insurance which will supplement lost income in the event of an accident or death. If you choose to enroll in employee coverage, this will be in addition to your employer provided Basic Life coverage.
- Basic Life/AD&D Benefit - Employer Paid
- Flat $10,000
- Voluntary Life/AD&D - Employee Paid
- Employee: Choose in $25,000 increments up to the lesser of 5x your annual salary or $150,000. Guaranteed Issue: $150,000
- Spouse: Choose in $5,000 increments up to the lesser of 100% of the amount you elect for yourself or $20,000. Guaranteed Issue: $20,000
- Child: Choose in $2,000 increments up to $10,000. Each eligible child must have the same amount of insurance. Guaranteed Issue: $10,000
- EOI (Evidence of Insurability): When you are first eligible (at hire) for Voluntary Life and AD&D, you may purchase up to the Guaranteed Issue (GI) for yourself and your spouse without providing proof of good health (EOI). Annually, you are able to increase elections 1 increment up to GI without proof of good heath.
- Voluntary Large Amount AD&D - Employee Paid
- Employee (Single coverage):
- $50,000 minimum up to $200,000 in $50,000 increments
- Family coverage:
- $100k EE+ $50k Spouse + $10k Child
- $150k EE + $75k Spouse + $15k Child
- $200k EE+ $100k Spouse + $20k Child
- EOI (Evidence of Insurability): No EOI required. you can elect any level for you and your family
Who's Your Beneficiary? Naming a beneficiary is a crucial part of electing life insurance. Also, don't forget to update your primary or secondary beneficiary if you experience a life event, such as a divorce or birth of a child.
Short Term Disability-Employer Paid
Everyday illnesses or injuries can interfere with your ability to work. Even a few weeks away from work can make it difficult to manage household costs. Short Term Disability coverage provides financial protection for you by paying a portion of your income, so you can focus on getting better and worry less about keeping up with your bills.
- Benefits begin on the 1st day for an accident or the 8th day for an illness (including pregnancy)
- Benefits are payable for up to 13 weeks
- Your benefit amount is 60% of your weekly earnings to a maximum of $750
Typical claims would include: pregnancy, injuries, joint, back and digestive disorders
Long Term Disability-Employer Paid
Serious illnesses or accidents can interrupt your life, and your ability to work for months – even years. Long Term Disability provides financial protection for you by paying a portion of your income, so you have financial support to manage your disability and your household.
- Benefits begin on the 91st day of a qualifying accident or illness
- If approved, benefits pay after you have exhausted your short-term benefit
- Benefits are payable until your retirement age; if you are still working beyond your retirement age and suffer from a qualifying accident or illness, a benefit will be payable for a specified amount of time depending on your age
- Your benefit amount is 60% of your monthly earnings to a maximum of $6,000
Typical claims would include: cancer, back disorders, injuries and poison, cardiovascular, joint disorders.
